A: You should make an appointment before you fill out your paperwork. Call (760) 242-7744 to make an appointment. Then you can download the forms found on the Forms Page.

A: Click on the Forms Page and follow the prompts. You will be able to fill out and print your Patient Information Form and Dental and Health History Form from the Forms Page.

A: Yes. You will need to print the completed form(s) and bring them with you. You will also need to bring in your insurance cards and driver license.

A: We currently cannot process payments through the web site.

A: You can request a call from the office from the Contact Page. The Contact Page will notify us during regular office hours to give you a call.

A: Yes, for some carriers. Go to the Insurance Page and click on your insurance company’s name or logo. Only the carriers with the logos can be accessed through our site.

A: Yes. Many of the procedures listed on the Procedures Page are linked to the American Dental Association’s informational web site. Or you can go directly to www.mouthhealthy.org to look up other topics related to dentistry.

A: You will sign your paperwork. We’ll make copies of your insurance cards and license. You will have necessary X-rays and an exam with the doctor.

A: No. The type of cleaning that you need will be determined at the time of your exam. Not all cleanings take the same amount of time or visits.

A: Yes. When you contact the office for the first time please let the staff know which doctor you want to see.
